Dear Ms. Testing,

Thank you for placing your trust in Mayo Clinic. We welcome the opportunity to be of service to you. We are pleased to confirm that an appointment has been scheduled for you. Your visit to the Mayo Clinic has been arranged on an outpatient basis. Your evaluations, tests and treatments will take place at the clinic. You will return to your place of lodging at the end of the day. Please refer to the attached hotel information and contact the hotel directly for reservations. If hospitalization becomes necessary, you will stay in one of the two Mayo Clinic hospitals in Rochester.

Length of Stay:
Prior to the initial consultation, it is difficult to predict the actual amount of time that you will be requested to stay. The average length of stay to complete a routine medical evaluation is 5 to 8 business days. Length of stay depends upon the number of tests and consultations needed. Our processes and systems are designed to be highly efficient. In some circumstances, your evaluation, surgery or treatment may involve a return visit.

Registration:
Please report to the International Center upon your arrival. The International Center is open from 6:30 a.m. to 5:30 p.m. Monday through Friday. Please note that Mayo Clinic is closed on the following US holidays in 2009: January 1st, May 25th, July 4th, September 7th, November 26th, and December 25th.

Making Your Financial Arrangements:

· A minimum deposit in the amount of $5,000 to $7,000 U.S. dollars is required at the time of registration.

  • This amount is a deposit toward services that you will receive and represents neither the minimum nor the maximum amount that you will be charged.

· During the course of your care you will be asked for an additional deposit if surgery, hospitalization, or extensive diagnostic testing is required.

  • Deposits may be made by major credit card, wire transfer, or cash.

· Final charges are based on actual services provided.

  • Overpayment received by check or cash will be refunded by mail via check payment.
  • Overpayment received by check or cash will be refunded via check. Overpayment received by wire transfer will be refunded by wire transfer.

· To make your financial arrangements prior to arrival or for information regarding financial matters, contact our International Finance Department at (507) 284-5063.

· Please contact your insurance company with coverage questions and to notify them of your appointments.

Estimate of Cost for Medical Services:
In order to provide an accurate estimate of cost for anticipated medical services it is necessary to first identify the specific test or procedure to be performed. These specifics are unknown until you are examined at Mayo Clinic. If you wish to obtain an estimate, please visit the International Center located in the lobby level of the Mayo Building, after your initial examination or call (507) 284-4024.

What to Expect at your Appointment:
Your Mayo Clinic visit will begin with a thorough evaluation of your medical concerns. Your primary provider will be responsible for coordinating your care and will order additional tests or consultations with specialists based upon your medical needs. After testing has been completed, your primary provider will review the results with you and make final recommendations.

Medical Record Information/ Materials to Bring:
If your diagnosis is biopsy proven, pathology slides (tissue fixed to the slide) are required. Failure to bring your pathology slides will result in delayed appointments. Copies of your current medical record, including pertinent x-rays and other test results, will also be helpful. Please be aware, however, that Mayo Clinic providers may find it necessary to repeat some of these tests to ensure the most accurate diagnosis possible.


Interpretive Services:
Mayo Clinic provides free interpretive services to non-English speaking patients during their clinic appointments. To arrange an interpreter, please notify our office.

Travel Arrangements:

· For assistance with lodging and travel arrangements, please consult the web site at www.mayoclinic.org/travel-rst, www.rochestermn.com, or you may book directly through our Carlson Travel Agent at 507-266-3392.

· When making airline reservations, make sure your destination is Rochester, Minnesota (RST).

  • Northwest and American Airlines fly to Rochester International Airport.

· Mayo Air is a service available to transport critically ill patients from anywhere in the world to Mayo Clinic. For information, please contact Mayo Medical Transport at Saint Mary’s Hospital at (507) 255-2808 or (800) 237-6822.

· A hotel list is included for your convenience. Please contact the hotel directly to arrange reservations.

· Concierge services are available to domestic and international patients.

  • The concierge is located in the International Center.
  • Services include transportation assistance, public notary services, information regarding local worship services, and general information on restaurants, banks, and a wide variety of local businesses.

· Patient Travel Services are also available and located on the Mezzanine Level of the Kahler Grand Hotel near the International Hospitality Room.

  • Services include airline and hotel room reservations, adjustment to travel itineraries or arrangement of ground transportation. These services do include a fee.

Travel Tips:
Travelers with medications and syringes should have all prescriptions in original containers, including the printed label from the dispensing pharmacy. During the airport screening process, please notify the screener if you have any implanted medical devices that may set off the metal detector.
